Existing Patient Re-registration

The practice joined BASS Medical Group in late August 2023.  Because of this, all existing patients prior to that date need to re-register.  We've tried to make this as painless as possible.  Just complete the form below for each child. When done, click the "submit" button.  You need to do this for each child separately (but auto-fill will make this easier.)
