New Patient Enrollment Information
Dear New 1-to-1 Pediatrics/Danville Pediatrics Patients and Families,
Welcome to the practice! We are excited to have you join our unique "hometown" practice.
Below are the electronic registration forms to sign your child(ren) up to be part of our practice. We are very proud to now offer all of the enrollment documents online in a HIPAA compliant electronic environment. Because each child has a separate chart in our EMR system, it is important that you complete the forms for each child. After completing all of the documents for one child, refresh the page and go through the process again for the next one. The "autofill" process should speed this up a bit.
Since opening in February 2011, 1-to-1 Pediatrics has prided itself on providing the highest quality, most personalized pediatric care available. Our unique “Low Volume” model allows us to spend more time with each of our patients during office visits and perform tasks such as referral completion, refills, prior authorization and completion of forms quickly and efficiently.
Spending extra time with patients and completing tasks that are not reimbursed by commercial health insurance takes up a good deal of the physicians’ time. To allow us to continue to work within this model, 1-to-1 Pediatrics charges an Annual Administrative Fee for each patient. As of June 1st 2022, the fee is $225 per patient/per year. If you sign up all of your children at once, the fee is capped at $550/year for families with 3 or more children. If you chose to sign up your children at different times then the annual fee will be $225 per child regardless of how many children you have. Also note, that if you sign up your children separately, you will need to use a different email address to register each child.
The Annual Administrative Fee pays for access to our After-Hours Advice Nurse Service, the time spent completing refills and prior authorization for certain medications, letter writing when needed, form completion for schools, camps and sports and a variety of other tasks which consume time but are not reimbursable by health insurance. The fee must be paid prior to scheduling your child’s first appointment.
After completing the enrollment forms for your child(ren), you will be directed to click on a link to pay the Annual Administrative Fee. You only need to do this once after you complete the enrollment documents for all of your children. Once at the Annual Administrative Fee website, enter the names and phone numbers of parents as well as the names of each child and their primary care physician in our office. It is important to enter the parents’ phone numbers correctly as doing so will enable families to access the after-hours advice nurse through these numbers. If your number changes or you would like to add phone numbers of grandparents or other caretakers, please let the office know.
Once family information is completed, you will be directed to pay the Annual Administrative Fee via credit or debit card. You will be charged a prorated amount for the remainder of this year. HSA cards may be used, however this may vary depending on your plan. Moving forward, you will be charged the annual fee for your family in early January of each year. Notification will be sent via text and email one week before the annual fee is automatically charged. If you move away or change providers during the year, please let us know so we can cancel your auto-renew. Also, if your card number changes, contact the office so we can update the information.

When completing this, please include ALL of your children and all phone numbers that might use to access the after-hours advice nurse service in the registration.
